Assess your current situation and options. Develop strategies and identify next steps. Facilitate successful transitions. Provide long-term supportive relationships.
 

Planning
Create strategies for safe and independent living. Look ahead to future needs.
 
Individual Support and Case Management
Assist individuals with advocacy, problem-solving, decision-making, goal setting, follow-through and coaching on life skills. Coordinate services.


Family Support
Negotiate expectations, goals and rules among family members. Facilitate communication and create a buffer to prevent conflict. Act as local eyes and ears-- visiting, monitoring and advocating for a loved one.


Referrals
Identify and connect individuals with public and private services and agencies. Assist with application and placement processes.


Health Care and Insurance
Navigate health care systems and health insurance, develop treatment teams and secure appropriate medical and mental health care.


Public and Private Benefits
Apply for and coordinate benefits, including Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), Medicaid, Medicare, and SNAP (formerly Food Stamps). Apply for short and long term disability insurance.


Transition Planning
Coordinate discharge from hospitals and rehabilitation facilities to home or other placements.


Turning 22
Simplify school-aged child’s transition to “adult services”.


Workplace Support
Determine need for changes based on disability. Clarify rights and responsibilities under the Americans with Disabilities Act, strategize about seeking accommodations. Assist with requesting and taking a leave.
